Physical Therapy

We recommend arriving at our facility at least 10-15 minutes prior to your scheduled appointment to go over any required paperwork and check in. We have changing rooms, bathrooms, and cubbies for your convenience.

Parking at Experience Momentum in Lynnwood is free and located onsite. We recommend arriving 20-30 minutes early as the parking lot can fill up during busy times, typically 5PM or later.
Parking at Experience Momentum in Seattle is limited on site but is available on the street or at additional pay lots within 1-2 blocks of the facility. Street parking is both 1-Hour and 2-Hour. Please pay attention to signage. We recommend arriving 20-30 minutes prior to your appointment if you are not used to city parking or the Fremont area.
Parking at Experience Momentum in Mill Creek is free and located onsite. This is a shared lot with multiple businesses, including a childcare facility that is quite busy during the drop off and pick up times, please plan accordingly to give yourself enough time if you need to park in an adjacent lot and walk over.

Please wear comfortable, active clothing so you can move easily. Your Physical Therapist may advise you of additional clothing adjustments after your initial visit if they deem them necessary for your continued treatment.

If we did not obtain a copy of your current insurance card and identification card prior to your appointment, we will require these items so we can scan them into your medical account. Some insurances require a prescription to be on file for your care in the state of Washington, if this is the case, we will ask you to provide a copy for us.
In addition, if you were unable to fill out any clinical online forms sent to you via email, you will need to fill those out on site prior to your appointment. If you would like these forms to be re-sent to your email, contact us at info@experiencemomentum.com

All initial visits last up to 53 minutes in length which includes an interview and an evaluation of your concerns and needs.
Depending on your needs, your Physical Therapist will provide personalized treatment interventions, create a home exercise program, and/or prescribe additional treatments by other providers either on your first or second visit. Please note that home programs are built from scratch and customized to your condition.

We offer external pelvic floor therapy that can address muscle weakness, leakage, and pelvic/low back discomfort. People of all sexes develop pelvic floor dysfunction for a variety of reasons. Whether you are feeling the effects of a pregnancy 3 months ago or 30 years ago, have been struggling with bladder/bowel issues, or have nagging pelvic/low back pain, addressing your pelvic floor can improve your experience in the gym and in life. Our PTs look at pelvic floor strength and coordination and teach you how to integrate that training to get you back to redefining what's possible.
Nutrition

During the intake session, your dietitian will deep dive into your nutrition history to learn about your habits and, more importantly, you as a person. We like to think of this session as an opportunity to get to know each other and explore the nutritional needs or concerns that brought you to Experience Momentum.
You can expect questions about your eating patterns, medical history (if appropriate) and your relationship with food and your body. Each session will conclude with you and your dietitian working together to set goals that will be addressed at follow-up sessions.

You'll need to complete all intake forms provided by our customer relations team and if your insurance requires, provide us with a referral and/or any necessary medical documentation from your physician. These may include any recent lab work, chart notes or diagnostic reports.

We take an individualized approach with each client and depending on your reasons for seeking guidance from an RD. Follow-up sessions can be anywhere from 30 - 90 minutes. Typically, our clients see us weekly or every other week at first and then, depending on your needs, we may space out appointments as appropriate.

Many insurance plans cover nutrition counseling in full or in part, depending on your needs or health concerns. We encourage our clients to call their insurance carrier member benefits service line to better understand your health insurance coverage.
While many plans cover nutrition counseling sessions at no cost to you, it is important to know your coverage prior to starting with us, as it may dictate how long sessions can be or how many sessions are covered.

Yes! Many of our clients live outside of the Fremont/Lynnwood area and prefer to see us virtually through Zoom, our HIPAA compliant telehealth platform.
If you do not live in the state of Washington, you may still be able to receive care from one of our dietitians depending on the state in which you live. Please call one of our clinics or complete our inquiry form to learn more about whether your state allows you to see a dietitian in another state.

Your dietitian may discuss your plan of care, response to treatment and other related aspects with other members of your care team, including our Physical Therapists, Massage therapists and Fitness Coaches and Trainers.
For example, if you already receive care from one of our dietitians and experience an unrelated injury or require surgery, they may recommend you visit one of our Doctors of Physical Therapy (DPT) for treatment.
We believe this allows us to provide the highest standard of care because our clinicians work as a team to provide personalized treatment and/or guidance based on your needs.

Registered Dietitian Nutritionists (RDN's) are experts in food and nutrition science. While all dietitians are nutritionists, not all nutritionists are dietitians. There are no educational or licensing requirements to use the term nutritionist, but there are educational and licensure requirements to become an RDN.

We use a weight inclusive approach when working with our clients and believe in respecting and uplifting body diversity. Our goal is to help our clients make peace with food, movement, and their body –and we do so by creating a safe space for all people, regardless of their size.

We believe that all foods fit and do not endorse diets specifically for the purpose of weight loss. Instead, we support our clients in reducing food-related stress as restrictive diets can increase stress, anxiety and preoccupation with food.
However, when treating a medical condition (Celiac Disease, Inflammatory Bowel Disease, certain Auto-Immune diseases etc.), our Registered Dietitian Nutritionists use evidence-based practices to support our client’s health and wellbeing by exploring how the body reacts to certain foods.
Massage Therapy

Our Fremont and Lynnwood clinics have multiple cash-pay options, including 60-, 90- and 120-minute appointments depending on availability and massage therapist.
In addition to single massage purchase options, we also have 4-, 6-, and 8-packs that are good for one calendar year from the purchase date and monthly wellness plan options that are on a recurring payment basis. Please get in touch with us for our current pricing.
Fitness

CrossFit: Constantly varied functional movements performed at a high intensity (courtesy of CrossFit, LLC). This means that workout stimuli are strategically varied with the intention of being prepared for whatever life throws at you. Movements are selected to be representative of the challenges of everyday life, and performed safely at a high intensity relative to each individual and their goals.
Bootcamp: High intense interval training that incorporates dumbbells and kettlebells. We are a physical therapy backed gym that focuses on functional fitness, multi-planar movements, and injury prevention.
Flexibility & Mobility: This class offers a space for recovery and accessory motions that utilize different techniques to enhance workouts or day to day living. This class is great for individuals that want to keep their bodies in check to combat sitting, repetitive motions, and the daily demands of life. Backed by our physical therapy team we will learn about different types of stretching, mobility tools, and how to find stability while working to decrease risk of injury.
Strength Training for Runners: Strength Training for Runners is a 55 minute class that includes strength exercises and aerobic conditioning to address demands specific for endurance and running athletes. This class is programmed by Experience Momentum Physical Therapists using evidence-based principles to keep runners out of the clinic and on pace to their next PR.
Additional Specialty Classes: Occasional seasonal or specialty classes offered such as Winter Conditioning, Barbell Basics or holiday WODs.

For your first class you will check in and meet your amazing coach who will ask you about any prior injury or modifications they should be aware of so they can assist you throughout the workout and answer any preliminary questions.
Once class has started members will participate in a warm-up and receive an explanation and demonstration of the WOD (workout of the day) before beginning the workout. During class, your coach will be monitoring and encouraging all participants, guiding the workout. Class will end with some form of cool down and check in. You should bring water/hydration for class, we do have fill up stations on site.

Mindbody is the app you will use to schedule classes and personal training appointments with us. Download the MindBody app onto your phone or visit the MindBody website and search for Experience Momentum. Note that this app does default to searching your nearby locations and therefore you will want to check and ensure the location you are signing up for is correct.

Yes! All three of our locations are bookable with ClassPass, class times and availability vary depending on the location and popularity.

Our Group Class programming is released through the app SugarWOD. This is a great way to see the upcoming workouts, track your progress, and stay connected to your new FitFam Community. Download the SugarWOD app and create a profile, make your Home Gym Experience Momentum and find our "Workout of the Day" posted.

We understand schedules and traffic are often hard to predict but for safety reasons and flow of the class we cannot allow entry into classes 10 mins into start time. Please note that if you are not present at the start of class time and there is an active wait list, your spot may be given up to any one who is presenting and waiting for entry.

Cost & Insurance
How Does Billing Work?
After your visit, Experience Momentum submits a claim to your insurance company using the information you provide at registration and the charges deemed by your provider for the visit.
Your insurance company will then process the claim based on your benefit plan, ask for additional information prior to processing if needed, or deny the services. Often times it can take around 30-45 days (about 1 and a half months) for insurance companies to process a claim and send you what is called an explanation of benefits
Some Things to Note:
If we are classified as a preferred provider or a provider who has a contract with your health insurer there will be an adjusted amount on your visits. This amount is not a payment, but rather a write-off or “reduction.” It is based on the contract in place between your provider (us) and your insurance company. Neither you nor the insurance company pays this amount. The provider essentially writes it off (which is why it is sometimes called the provider’s responsibility).
Patient Responsibility may be labeled “Deductible,” “Copay,” “Coinsurance,” or “Patient Pay.” It is the amount that you, the patient, are responsible for paying. If you have not met your deductible for the year, you must pay costs up to the deductible amount before insurance will pay. Once you have met your deductible, you may be responsible for a portion of the allowed amount. This is called coinsurance. Some health services do not have coinsurance and/or deductibles.
Once your visit/s have been processed, Experience Momentum’s Billing team will send you a statement for the amount you owe. Your cost includes any unpaid fees, copays, your coinsurance responsibility and any remaining amount up to your annual deductible or “out-of-pocket maximum” that you may owe. If your insurance plan pays the full allowed amount, you will not receive a bill.
How Much Will I Owe?
Experience Momentum does not have the ability to pre-determine your out-of-pocket cost. We allow your insurance to process your claim based on the plan you hold with them.
Here are some things to help you navigate:
Most insurance companies offer several different plans or subsidiaries. Thus, two patients with Blue Cross Blue Shield, for instance, may have completely different benefits, and therefore, completely different financial responsibilities. Some plans have no copays or deductibles; others may have a $10,000 deductible. Furthermore, some providers may not accept all plans from a particular insurance. This is why it is crucial that you investigate the details of your specific plan.

There are various reasons why your insurance did not make a payment on your bill. You may have a deductible still due, or there could be a benefit limitation that is causing payment to be withheld. If you are using insurance and need assistance understanding your patient responsibility or why a visit processed the way it did, please call your insurance carrier first. Typically, you can find a member services number on the back of your insurance card. We process your claims through your carrier, and they determine what you owe based on your plan.
